Liability Waiver and Release Form

Notice: Please read this entire disclaimer carefully before signing. By signing this waiver, you give up certain legal rights.

This agreement releases Little Plaza from all liabilities relating to injuries, products liability, or other loss or damage to person or property suffered by me, my child, or any minor participant, arising in whole or in part from participation in any activities or events hosted at Little Plaza or organised by Little Plaza outdoor.

By signing this agreement, I agree to hold Little Plaza entirely free from any liability, including financial responsibility for damages or injuries incurred, regardless of whether injuries are caused by negligence. I understand and acknowledge that participation in activities at Little Plaza may involve inherent risks, including but not limited to, slips, falls, cuts, bruises, and other injuries.

I, as the parent or legal guardian of a minor participant, or as an adult participant, will be responsible for supervising and ensuring the safety of the minor participant(s) during their time at Little Plaza. I understand that it is my responsibility to inform Little Plaza of any participants with allergies or other medical conditions prior to the event.

Activities: Activities offered by Little Plaza may include but are not limited to, Yoga sessions (indoor and outdoor), games, entertainment, and recreational activities suitable for children's birthday parties for example. Participation in these activities may involve various equipment, materials, and props.

Risks: These risks include but are not limited to, minor injuries such as bumps, bruises, and cuts, as well as more serious injuries such as sprains, strains, or fractures. For pregnancy sessions, responsibility is with the participants themselves to consult with the medical specialist if it's safe or not to participate certain activities, such as pregnancy mindful yoga indoor and outdoor sessions, outdoor walks these outdoor activities. I understand that Little Plaza will take reasonable precautions to ensure the safety of participants, but accidents or injuries may still occur.

I acknowledge that I have read and understood the terms of this waiver and release form, and I voluntarily agree to its provisions.
